What We Do
The Multi-County Medical Outreach Clinic (MCMOC) is a free walk-in clinic in Columbia City, Indiana, that provides quality healthcare to the uninsured and underinsured population in Northern Indiana.
Basic primary care
Treatment of acute illnesses (examples: urinary, respiratory, and skin infections)
Treatment of chronic health conditions (examples: hypertension, diabetes)
Patient education
Services Provided
Disease prevention & screening
Physical exams for employment, college or special programs (We do not perform physicals for disability claims)
Women's Health Screenings (excludes Pregnancy or Birth Control )
Men's health
Referrals for laboratory testing and x-rays
Specialty referrals
Referrals for: Optometry, Dental Work
The Clinic will do as much as possible to provide the needed care for our patients. If the Clinic cannot provide the services needed we will make arrangements with outside agencies to arrange a referral for further care. The Clinic will work with these agencies to offer the patient services at no cost or at a reduced fee, but we are not able to guarantee these terms.

Plan Your Visit
You do not need to make an appointment. Patients are accepted on a first come, first serve basis. When you come in, we will talk about what is concerning you and then create a plan to help you get on the path to better health.

The clinic is closed on federal holidays. We are located inside the Parkview First Care building: 885 W Connexion Way, Columbia City, IN 46725.

More Than Just Prescribing Medicine
A visit at the MCMOC Free Clinic is about giving you the tools to live a healthier, longer life.
At your visit, you will meet with licensed professional caregivers: doctors, nurse practitioners, and nurses. They are volunteers who are committed to serving our community through free healthcare.
Originally Wolf Lake Free Health Clinic, we moved to Columbia City in 2011 and started using the name Multi-County Medical Outreach Clinic.
